Randomized Text - What is it and Who Needs it?
With regards to computerized reports, "Randomized Text" is simply a phrase which describes text that a computer program spits out, that looks a little or a lot different from one report to the next report, with the idea being that your reports will never look quite the same, despite how much has changed or not changed with the patient.
Now this may have had some validity in "the old days of computers" (15 years ago), when most doctors were still using typewriters or writing their Narratives and Daily Notes by hand, except for a handful of geeky doctors who were savvy enough to see computers as great office tools.
Way back then the insurance adjuster or attorney might well have complained to one of those geeky doctors that, "Your reports look canned," or "Your reports all look the same," etc., etc. on and on. In other words, "It looks like you used a computer to write your Reports." How ghastly!

Back in the Old Days...
So, that experience may have left us with some sort of an idea that we had to hide the fact that we were putting our computers to a more productive use than merely playing Video Ping Pong.
And for your Narratives and Daily Notes not to have been mistaken for fraudulent pieces of pure fiction, they had to be written by hand, or produced by hunting, pecking and pounding them out on a 50 lb. Royal typewriter, decorated with little globlets of White Out. Or at best, for the more high-tech doctors, an IBM Selectric Typewriter. But, computer assisted reports? No way!

Confessions of a Software Company Owner
So, I must confess, some 8 years ago I gave in to the Sales Reps' demands and the malicious rumors that Report Master MUST have randomized text to survive.
Many, many programming hours later, Report Master ended up with, and still currently utilizes one of the best, if not the best, text randomizer on the market. And I have no intention of getting rid of it. But, to this day, I still believe what I believed 9 years ago when I first began getting nagged about randomized text. And that is simply this: you don't really need randomized text in a report writer.
Now, if you're writing the Great American Novel or the next million dollar screenplay, the last thing you'll want is predictable, robotic, or "canned" language. But as far as Narrative Reports and Daily SOAPs go, it should be, "Only the facts maam. Only the facts."

We Actually Have Entered the 21st Century!
Today, computers are very much a fact of life. Any insurance adjuster who actually believes that you didn't use a computer to help you write your Narratives and SOAP Notes, even if you only used Microsoft Word, is a clueless jackass.
The bottom line is this: If the reason your Narratives and SOAPs are all looking the same is because your patients aren't getting any better and basically aren't experiencing any changes, then randomized text is the least of your troubles.
But, assuming that you are bringing about positive changes in your patients, all you need to concern yourself with is making sure those positive changes are fully documented, and communicated in a clear, understandable manner.
If some insurance adjuster rejects a claim based on your reports looking "canned," tell him his rejection letter looks canned and ask him or her for the real reason for the rejected claim.
